FIRST DAY OF SCHOOL PACKET ONLINE 2017-2018 

Back to School REQUIRED Document form

Please review all information in all parts 1-6, then sign and return this form to your school office along with any other required or optional forms by August 30, 2017.

PART 1:  Back to School REQUIRED Document form, Annual Notice to Parents English and Spanish

PART 2: OAUSD Policies regarding: Bus Conduct, Student Non Discrimination/Harassment, Compulsory Attendance/Truancy

PART 3: Pesticide Notification, Resolution on Student/Family Rights, Policy Classroom Visits, Uniform Complaint Procedures, Williams Complaint Procedures

PART 4: Student Internet Use of Technology

PART 5 RETURN REQUIRED:   Student Release/Emergency Information Card, Free/Reduced Lunch Application. The Free/Reduced Lunch application is required whether you qualify or not. If you do not qualify for eligibility please just write "Not Eligible" on the form with the student's name on it. Free/Reduced Lunch Application. The application was sent home in our Back to School mailing but is also available at all school sites and at the District Office.

PART 6 OPTIONAL FORMS, RETURN IF APPLICABLE:Volunteer Application,Bus Pass Application, Student Health History,  Media Opt Out (The Media Opt Out form is only required if you do NOT want photos, videos, recordings, etc of your child released. If you do not sign and return the Media Opt Out form you are giving permission for photos, etc of your child to be released to the media.) Authorization for Administering Medication

Hard copies of this packet are available for viewing at the District Office and at each school site office.

Please return all required forms by August 30, 2017 to your school office.
