Please review all information in all parts 1-6A, then sign and return this form to your school office along with any other required or optional forms by August 30, 2017.

PART 4: Student Internet Use of Technology agreement to be signed by and returned to your school office by student and parent
PART 5: MEALTIME and Free/Reduced Lunch InformationThe Free/Reduced Lunch application is required whether you qualify or not. If you do not qualify for eligibility please just write "Not Eligible" on the form with the student's name on it. Free/Reduced Lunch Application. Applications are available at all school sites and at the District Office but may also be filled in and submitted through MEALTIME.
PART 6 OPTIONAL FORMS, RETURN IF APPLICABLE: Volunteer Application, Media Opt Out (The Media Opt Out form is only required if you do NOT want photos, videos, recordings, etc of your child released. If you do not sign and return the Media Opt Out form you are giving permission for photos, etc of your child to be released to the media.) Authorization for Administering Medication
PART 6A: OPTIONAL FORMS, RETURN IF APPLICABLE TO YOUR FAMILY: Bus Pass Application, Authorization for Administering Medication, Voluntary Parent Drive Form
