Kindergarten registration and enrollment for all grades in the Old Adobe Union School District will be held at the schools this year. Registration will begin on January 11, 2010 at the locations and times below:
9:00am-4:00pm
Please bring to registration your child's immunization records, and an original birth certificate or any other legal document (passport, baptismal certificate or official hospital record) for proof of birth. Copies will be made at the school. Proof of Residence will be required and may be verified by a utility statement (gas and electricity, telephone, water, waste management), or Renter's/Buyer's Agreement from the last 45 days. The Education Code and Old Adobe Board Policy states that, to enter Kindergarten, a child must be 5 years old on or before December 2 of the year they enter school . We have registration forms available for you to fill out at each of our schools. However, if you choose to fill them out ahead of time we have included them below for your convenience. Please note, Inter-district (transfer between districts), Intra-district (transfer within district), Employment Related transfer, and Student Registration forms must be filled out in triplicate. If you are planning on attending your school of residence, you will need to fill out a Student Registration Form , an Oral Health Assessment/Waiver Request Form , and a Student Health History Form .
If you are planning on attending a school other than your school of residence, you will also need to fill out an Intra-District Transfer Request (to attend another school within the district), an Inter-District Transfer Request (to attend one of our schools if you do not reside in the Old Adobe Union School District), or an Employment-Related Transfer Request (to attend a school in the district in which your employment is located).
